RMA Procedures:

  • All fields of the RMA form must be completed for an RMA authorization number. Please complete the RMA form with detailed descriptions of the problem with your product.
  • Fax the completed RMA form with a copy of the purchase invoice to (905) 789-7062
  • Either an RMA number will be faxed back to you or a reason for RMA denial.
  • Your assigned RMA number will be valid for 30 days from the date of issue.
  • You must agree to the term and conditions above before submitting this form.
